DOCUMENT CHECKLIST FOR VISA APPLICATION

Passport: Original Passport with validity of minimum six months and minimum two blank page for visa stamp.

1. Copy of the first and last page of your current valid passport. If you have any observations on your passport please submit a copy of the observation page.
2. Attach all your old passports (if any).

Photo specification: 3 photographs – 35 x 45 mm, matt finish, white background, without border, 80% face coverage, clear forehead and ears. If wearing spectacles, eyes must be clearly visible without any glare on glasses.

Covering Letter: Covering-Letter from applicant on business letter head mentioning name, designation, passport number, purpose and duration of visit in brief. The letter should be duly signed by authorized signatory with company stamp and addressed to – Consulate General.

Please Note: Covering-Letter on Letterhead if applicant is Self Employed or on plain paper if employed.

Financials 

1. a) Company and Personal ITR for 3 yrs. 
2. b) Salary slips for the last six months. 
3. c) Original Company and Personal Bank Statement for last six months mentioning the Bank’s name, Bank’s Telephone Number clearly 
4. d) Credit Card copy/Foreign Exchange endorsement 
5. e) Company’s Balance Sheet 
6. f) Property papers/Fixed Deposits/Investments.

Proof of Occupation 

1. a) Company Registration Certificate / Certificate of Incorporation 
2. b) Brief Company Profile 
3. c) Articles of Memorandum (In case the applicant is MD/Director) 
4. d) Proof of Proprietorship / Partnership (In case applicant is Proprietor / Partner in a firm) 
5. e) Import / Export License (if applicable) 

Employed: 

1. a) Copy of Appointment letter/Leave certificate from the Employer. 

Student:

1. a) Leave Sanction Letter and ID card copy from school 
2. b) Parents have to provide ITR, Bank statement and NOC letter.

Tour Detail: Airline Reservation: Ticket Itinerary. Hotel Booking: Hotel confirmation and Tour Confirmation.

For Sponsored/Invitation Based Visa

The sponsor has to furnish the following documents: 

Invitation Letter: 

1. Invitation Letter from inviter or Sponsorship letter from the sponsor stating the declaration of support.
2. Proof of Sufficient Financial documents of the inviter staying in America. 
3. Identity Proof 
   1. Photocopies of relative’s proof of status e.g. Green card, valid visa etc.
   2. Bank statement of 3 months and last 6 months salary slip.
   3. Resident proof (water bill, gas bill etc)

Please Note: 

1. Personal appearance is Mandatory.
2. If you have temporary injuries on your fingers, please wait until the injuries heal before your appointment date. You should also remove any henna on your fingertips, or allow it to fade, as this may prevent from getting a clear scan.
3. For children under 5 years, application should be dropped in drop box at the Offsite Facilitation Center (OFC). Passport and visa copy of the parents should be attached with the application.
